The City of Middleton wants to be ready for your next event! We all know though that NO job is complete until the paperwork is done.  Please download the Special Events Form, look it over to determine what if any permits, etc. your event might require.  Then, if there are permits and notifications that may be necessary, fill it out electronically, print it and send it to the City of Middleton at 7426 Hubbard Avenue to give Middleton officials information about your upcoming event. The information you provide will help us to plan ahead. We want to ensure that Middleton is prepared to give your attendees the best possible experience.
